- Your most recent passport is undamaged and can be submitted with your application
- You were at least 16 years old when your most recent passport was issued
- Your most recent passport was issued within the last 15 years
- Your most recent passport was issued in your current name or you have legal documentation of a name change.
Complete form DS-82: Application for Passport Renewal. This form can be found on the US Department of State website. Fill out the form completely and accurately.
Gather supporting documents. You will need to include your most recent passport, a passport photo, and the renewal fee. If you have changed your name, you will also need to include legal documentation of the name change.
Submit your application. You can submit your application in one of the following ways:
- Mail your application to the address listed on the form.
- Use a delivery service (such as FedEx or UPS) to mail your application.
- Make an appointment at a US Department of State Passport Agency.
It is recommended that you renew your passport at least six months before it expires, as many countries require that your passport be valid for at least six months beyond your date of entry.
